The forms in this section can be completed on-line.  You MUST complete all required fields (marked with an *) or the submission will fail.

Facility/Equipment Usage Request Form

Parental Permission Form: This form is for use by parents to grant permission for any activity for which it is required at FBC.  If you have questions about the need for a permission slip, contact the sponsoring group.

Facility/Equipment Usage Request Form: If you would like to use the church or equipment for an activity, please use this form to make your request.  It must be submitted at least 1 week in advance of your activity.  All activities will be reviewed by the associate pastor and you will be contacted with the availability.

Small Group Meetings Report Form: Used by small group leaders to fill out monthly reports to send to the church for record keeping purposes.

Facility/Vehicle Use Policy: Outlines the basic expectations for those who utilize our facilities.

Statement of Agreement With Key Policy: Required for those who are issued keys.

Statement of Agreement with Facility/Vehicle Use Policy: Require for those who utilize facilities, equipment or vehicles.

Wedding Policy:  This document will give you the basic information for getting married in our church.  Please note, no weddings are scheduled without the approval of the Senor Pastor, or in his absence, the Associate Pastor.
